School Registration

AVA SCHOOL REGISTRATION FORMS

All families are required to complete the Douglas County School District's Express Check-in which opens July 24th. Express Check-in is where you update your household contact information and verify that everything is correct.
 
In addition to Express Check-in, AVA has its own forms that must be completed each year.
Please complete the following forms:
 
If your student will be walking/biking to/from school, you must complete the Walker form giving your student permission. Please complete one for each student. 
 
If you plan on picking up your student in carpool, you do need to request a new CARPOOL TAG for this school year (Every school year they are a different color for safety reasons). Please complete ONCE per family.
 
Every family must complete this form (one entry per family) acknowledging that you have read the Parent Student Handbook and agree to its content. The Parent Student Handbook can be viewed here
 
If you plan on volunteering at the school during the school year, you will need to complete this form. It must be completed every year, one per volunteer/family.
 
This form is optional -- you only need to submit it if there are sites/apps on the AVA Approved Apps and Website list you do not want your student to use. 
 
This form must be completed for every elementary school student. 
 
TECHNOLOGY AGREEMENT FOR MIDDLE SCHOOL PARENTS
This is a paper form that will be sent home with students on the first day of school. Students and their parent(s) will be required to review, sign, and return the form. If you need to print a new copy of the form, use this link
 
SCHOOL DIRECTORY OPT-OUT FORM
Complete this form if you do not want your household information included in the AVA directory. Only one form submission per household is required. No longer accepting responses. Please email [email protected] with any questions or additions. 
 
STUDENT EMERGENCY CARD
The school must have a completed Student Emergency Card for each student. Teachers will be sending a form home with your student. You can also print it by clicking the document below.
